В литературе все чаще описывается нетипичная клиническая картина псориаза и атопического дерматита (АД) у детей, сочетающая в себе черты обоих заболеваний. Гистологическое исследование, проводимое в рамках комплексной диагностики, в таких случаях не дает однозначный ответ в пользу того или иного заболевания, представляя неопределенную морфологическую картину в сравнении с классическими формами псориаза и АД. При этом классические морфологические критерии для псориаза и АД были описаны без учета возраста пациентов, поэтому остается неясной возможность применения морфологических критериев взрослых для диагностики патологического процесса в коже детей.</p></sec><sec><title>Ц е л ь</title><p>Ц е л ь . Дать характеристику патоморфологическим изменениям в воспалительных элементах кожи детей с неопределенной клинической картиной (НКК) заболевания в сравнении с изменениями в воспалительных элементах кожи при типичной клинической картине псориаза и АД у детей и взрослых.</p><p>М а т е р и а л ы и м е т о д ы . Было проведено сравнительное морфометрическое исследование биоптатов кожи детей и взрослых 6 групп: 2 группы взрослых пациентов – с псориазом и АД; 2 группы детей с псориазом и АД, имеющими типичную клиническую картину, подтвержденную гистологическим исследованием; группа детей с НКК заболевания (отсутствовали типичные морфологические признаки псориаза и АД); последнюю, 6-ю группу составили биоптаты кожи детей, полученной при хирургических манипуляциях при недерматологических заболеваниях.</p></sec><sec><title>Р е з у л ь т а т ы</title><p>Р е з у л ь т а т ы . Сравнительный морфометрический анализ биоптатов кожи исследуемых выявил, что при типичном псориазе структурные изменения в эпидермисе у детей и взрослых имели наименьшее количество различий. При АД структурные различия элементов кожи детей и взрослых более выражены. Клеточность воспалительных инфильтратов в детских группах была ниже, чем у взрослых с соответствующими заболеваниями, и с большей долей лимфоцитов. В то же время элементы сыпи детей с НКК заболевания имели больше общих морфологических черт с элементами кожи детей других групп, чем с элементами кожи взрослых.</p></sec><sec><title>З а к л ю ч е н и е</title><p>З а к л ю ч е н и е . Структурные преобразования эпидермиса у детей с НКК заболевания по выбранным классическим критериям занимали промежуточное положение между группами псориаза и АД и имели больше общих морфологических черт с элементами кожи у детей. Однако выявленное своеобразие воспалительного инфильтрата в элементах кожи при НКК заболевания требует дальнейшего изучения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>I n t r o d u c t i o n</title><p>I n t r o d u c t i o n . The literature increasingly describes an atypical clinical picture of psoriasis and atopic dermatitis (AD) in children, combining the features of both diseases. Histological examination conducted as part of a comprehensive diagnosis in such cases does not provide an unambiguous answer in favor of one or another disease, presenting an undetermined morphological picture in comparison with the classical forms of psoriasis and AD. At the same time, the classical morphological criteria for psoriasis and AD were described without considering the age of patients, so it remains unclear whether morphological criteria for adults can be used in the diagnosis of skin pathological process in children.</p></sec><sec><title>A i m</title><p>A i m . To characterize the pathomorphological changes in the skin lesions of children with an undetermined clinical picture (UCP) of the disease in comparison with changes in the inflammatory skin lesions with a typical clinical picture of psoriasis and AD in children and adults.</p><p>M a t e r i a l s a n d m e t h o d s . A comparative morphometric study of skin biopsies of children and adults of 6 groups was performed : 2 groups of adult psoriasis and AD patients; 2 groups of children with a typical clinical picture of psoriasis and AD confirmed histologically; a group of children with UCP of the disease (without typical morphological signs of psoriasis and AD); the last, 6th group, comprised biopsies of children’s skin obtained during surgical procedures for non-dermatological diseases.</p></sec><sec><title>R e s u l t s</title><p>R e s u l t s . The comparative morphometric analysis of the skin biopsies showed that in typical psoriasis, the structural changes in the epidermis of children and adults were the least. In AD, the structural differences of the skin lesion morpho- logy in children and adults are more pronounced. The inflammatory infiltrate cellularity in the children’s groups was lower than in adults with the same diseases and had a higher count of lymphocytes. At the same time, the skin lesions in children with UCP had more morphological features in common with the skin lesions of children in other groups than with those of adults.</p></sec><sec><title>C o n c l u s i o n</title><p>C o n c l u s i o n . According to the selected classical criteria, the structural changes of the epidermis in children with UCP occupied an intermediate position between the psoriasis and AD groups and had more morphological features in common with the skin lesions in other children groups.
