Первичные Т-клеточные лимфомы кожи (ТКЛК) представляют собой гетерогенную группу экстранодальных неходжкинских лимфом c первичным поражением кожи. Диагностика ТКЛК часто затруднена, поскольку в дебюте злокачественной лимфомы имеются клинические и морфологические сходства неоплазии с такими неопухолевыми дерматологическими заболеваниями, как хронический экзематозный дерматит, псориаз, красный волосяной лишай или грибковые инфекции кожи. Кроме того, в отличие от В-клеточных неходжкинских лимфом Т-клеточная опухолевая пролиферация часто не имеет специфического иммунофенотипа, маркерных хромосомных аберраций и носит поликлональный характер, что в значительной мере осложняет использование иммуногистохимических и молекулярно-генетических методов в дифференциальной диагностике. Вместе с тем в патогенезе ТКЛК участвуют как генетические, так и эпигенетические факторы, в частности, микроРНК (миРНК).</p></sec><sec><title>Ц е л ь</title><p>Ц е л ь. Выявление специфического профиля миРНК опухолевого субстрата, позволяющего дифференцировать ТКЛК от доброкачественных образований кожи, а также определение роли миРНК в патогенетических механизмах развития заболевания.</p><p>М а т е р и а л ы и м е т о д ы. Методом полимеразной цепной реакции с обратной транскрипцией в реальном времени проведен анализ уровней экспрессии миРНК-126, -145, -125b, -203a, -99, -23a, -146a и let-7g в 10 фиксированных формалином и залитых парафином образцах биопсии кожи пациентов с диагнозом ТКЛК, а также 10 образцах биопсии кожи пациентов с установленным диагнозом псориаза и 20 образцах биопсии кожи пациентов с установленными диагнозом экземы, составляющих контрольную группу доброкачественных образований.</p></sec><sec><title>Результаты</title><p>Результаты. Сравнительный анализ уровней экспрессии миРНК в опухолевых (ТКЛК) и доброкачественных образцах показал статистически значимое снижение уровня экспрессии миРНК-203а и увеличение уровней экспрессии миРНК-146а и let-7g в опухолевых образцах (p &lt; 0,05). ROC-анализ показал, что let-7g и комплексный биомаркер, являющийся отношением уровней экспрессии миРНК-203а/миРНК-146а, являются высокочувствительными и специфичными маркерами для диагностики ТКЛК.</p></sec><sec><title>З а к л ю ч е н и е</title><p>З а к л ю ч е н и е. Анализ уровней экспрессии миРНК может быть перспективным инструментом для дифференциальной диагностики дерматотропных Т-клеточных лимфоидных гемобластозов и негемопоэтических болезней кожи.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>I n t r o d u c t i o n</title><p>I n t r o d u c t i o n . Primary cutaneous T-cell lymphomas (CTCL) are a heterogeneous group of extranodal nonHodgkin’s lymphomas with primary skin involvement. The diagnosis of CTCL is often challenging because, in the early stages, malignant lymphoma can have clinical and morphological features that are similar to benign dermatological conditions such as chronic eczematous dermatitis, psoriasis, pityriasis rubra pilaris, or fungal skin infections. Additionally, unlike B-cell non-Hodgkin’s lymphomas, T-cell tumor proliferation often lacks specifi c immunophenotypic markers, characteristic chromosomal aberrations and tends to be polyclonal, which signifi cantly complicates the use of immunohistochemical and molecular genetic methods in diff erential diagnosis. However, both genetic and epigenetic factors, microRNAs (miRNAs) in particular, are involved in the pathogenesis of CTCL.</p></sec><sec><title>A i m</title><p>A i m . To identify a specifi c miRNA profi le of the tumor substrate that can diff erentiate CTCL from benign skin lesions and determine their role in the pathogenetic mechanisms of disease development. Materials and methods. Using real-time reverse transcription polymerase chain reaction levels of expression of miRNAs-126, -145, -125b, -203a, -99, -23a, -146a, and let-7g were analyzed in 10 formalin-fi xed paraffi nembedded skin biopsy samples from patients with CTCL, 10 skin biopsy samples from patients with psoriasis, and 20 skin biopsy samples from patients with eczema.</p></sec><sec><title>R e s u l t s</title><p>R e s u l t s . Comparative analysis of miRNA expression levels between CTCL and benign skin samples showed a statistically signifi cant decrease in the miRNA-203a expression and an increase in the miRNA-146a and let-7g expression in tumor samples (p &lt; 0,05). ROC analysis demonstrated that the let-7g level and the composite biomarker, i.e. the ratio of miRNA-203a/miRNA-146a, are the highly sensitive and specifi c markers for CTCL diagnosis.</p></sec><sec><title>C o n c l u s i o n</title><p>C o n c l u s i o n .
