Развитие ГУС наиболее часто связывают с заболеванием энтерогеморрагическим эшерихиозом и шигеллёзом, однако на современном этапе появляются сообщения о новых бактериальных патогенах, которые могут быть ответственны за развитие данной формы ТмА. Одним из них является Campylobacter. Патогенез ГУС при инфекциях, вызванных Campylobacter, неизвестен, что не позволяет выработать адекватные подходы к диагностике, терапии и реабилитации данных состояний. В Новосибирске в 2018 г. зафиксировано два случая ГУС у детей, когда в качестве этиологического агента был выявлен Campylobacter. Приводим описание данных случаев, которое демонстрирует сложность ведения пациентов и высокую ургентность ГУС, ассоциированного с Campylobacter. Проведенный анализ вызывает сомнение в том, что развитие ТмА при кампилобактерных инфекциях опосредуется токсином Шига, однако не позволяет исключить как нейраминидазный, так и аутоиммунный генез развития подобных состояний. Таким образом, ГУС, развивающийся при кампилобактерной инфекции, требует дальнейшего изучения с целью выработки и оптимизации подходов к его диагностике и терапии.</p></abstract><trans-abstract xml:lang="en"><p>Hemolytic uremic syndrome (HUS) is a special form of thrombotic microangiopathy (TMA) characterized by obligate kidney damage. The development of HUS is most often associated with enterohemorrhagic escherichiosis and shigellosis, however, at the present stage there are reports of new bacterial pathogens that may be responsible for the development of this form of TMA. One of them is Campylobacter. The HUS pathogenesis in infections caused by Campylobacter is unknown, which does not allow us to develop adequate approaches to the diagnosis, treatment and rehabilitation of these conditions. In 2018 there were recorded two cases of HUS in children in Novosibirsk, Campylobacter was identified as the etiological agent. These cases are described in the article. The presented description demonstrates the complexity of patient management and the high urgency of Campylobacter-associated HUS. The analysis makes the Shiga-toxin mediated mechanism of TMA development in campylobacter infections doubtful, however, it does not allow to exclude both the neuraminidase and autoimmune genesis of such changes.
