Определение клинико-лабораторных особенностей клещевых риккетсиозов, в частности риккетсиозов, вызванных Rickettsia (R.) raoultii и R. sibirica, у взрослых жителей Новосибирской области.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В период с апреля по сентябрь 2016-2017 гг. обследованы 665 чел., госпитализированных в ГБУЗ НСО «Городская инфекционная клиническая больница № 1» и имеющих в анамнезе присасывание или наползание клеща или эпизод выезда в лесную зону с последующим появлением лихорадки с температурой от 37 °С и выше. Образцы крови, спинномозговой жидкости и соскоба кожи с места присасывания клеща брали от пациентов в первые 1-2 дня после госпитализации и перед началом антибактериальной терапии. Всем пациентам проводили комплексное клинико-лабораторное обследование на догоспитальном этапе и в стационаре. ДНК риккетсий выявляли методом двухраундовой ПЦР с последующим секвенированием ПЦР-фрагментов.</p></sec><sec><title>Результаты</title><p>Результаты. ДНК риккетсий обнаружена в клинических образцах у 37 пациентов (5.6 % от числа всех обследованных): у 14 пациентов — ДНК R. sibirica, у 15 — R. raoultii, у одного — ДНК «Candidatus R. tarasevichiae», у 7 пациентов — ДНК других видов и геновариантов риккетсий из группы клещевой пятнистой лихорадки (КПЛ). Симптомы болезни, вызванной R. raoultii, отличались от симптомов сибирского клещевого тифа (СКТ). Только у двух из 15 больных (13.3 %), в образцах которых выявлялась ДНК R. raoultii, выявлен первичный аффект с регионарным лимфаденитом и сыпью, в то время как у пациентов с СКТ — у 11 из 14 (75.8 %). У всех пациентов с R. raoultii отмечены астенический синдром, у двух третей — головная боль и фебрилитет, у каждого третьего — фебрилитет до 39 oC Редко отмечались артралгии и миалгии, характерные для СКТ. Явления менингизма отмечены у 7 пациентов с R. raoultii и у 4 пациентов с R. sibirica, причем у 5 пациентов ДНК риккетсий обнаружена в спинномозговой жидкости — у 4 больных ДНК R. raoultii и у одного — ДНК R. sibirica. Длительность лихорадочного периода в условиях проведения антибактериальной терапии составила в среднем 5 дней.</p></sec><sec><title>Заключение</title><p>Заключение. Риккетсиозы, вызванные R. raoultii, не имеют патогномоничных специфических признаков, хотя иногда выявляются симптомы, которые позволяют предположить диагноз, такие как лихорадка, сыпь, лимфоаденопатия и первичный аффект.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Determination of clinical and laboratory features of tick-borne rickettsioses, in particular caused by Rickettsia (R.) raoultii and R. sibirica, in adult residents of the Novosibirsk Region.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. In the period from April to September 2016-2017 a total of 665 patients, who were examined on admission to the Novosibirsk Infectious Clinical Hospital No. 1 and had a history of tick attack, or crawling, or an episode of visiting the forest zone with the subsequent rise of body temperature to 37°С and above. Samples of blood, cerebrospinal fluid and skin scrapings from the tick’s bite site were taken from patients in the first 1-2 days after hospitalization and before the antibacterial therapy start. All patients underwent a comprehensive clinical and laboratory examination at the prehospital and hospital stages. The rickettsial DNA was detected by a nested PCR method followed by sequencing of the PCR fragments.</p></sec><sec><title>Results</title><p>Results. The rickettsial DNA was found in clinical specimens of 37 patients (5.6% of all examined patients): in 14 patients — the DNA of R. sibirica, in 15 — R. raoultii, in one patient — Candidatus R. tarasevichiae, in 7 patients — the DNA of another rickettsia species and genovariants of the spotted fever group (SFG). The symptoms of R. raoultii disease were different from those of Siberian tick-borne typhus (STT). Only two of 15 patients (13.3%), in whose samples the DNA of R. raoultii was detected, had an eschar with regional lymphadenitis and rash, while in patients with STT — in 11 of 14 (75.8%). All patients with R. raoultii had asthenic syndrome, two-thirds — headache and febrile fever, and one-third — febrile fever up to 39°С. Arthralgia and myalgia characteristics of STT were rarely noted. The meningism phenomena were observed in 7 patients with R. raoultii and in 4 patients with R. sibirica, and in 5 patients the rickettsial DNA was found in cerebrospinal fluid — in 4 patients with the DNA of R. raoultii and in one — with that of R. sibirica. The duration of the febrile period under conditions of antibacterial therapy was 5 days on average.</p></sec><sec><title>Conclusion</title><p>Conclusion.
