В настоящее время установлено, что ГЭБ является высокоактивной структурой. С одной стороны, он защищает ткань мозга от химических, физических и иных воздействий, а с другой — уникально приспособлен для передачи сигналов между ЦНС и другими частями организма. Клетки ГЭБ реагируют на сигналы, поступающие из ЦНС или компартментов крови, которые могут стимулировать изменения в их барьерных, транспортных и секреторных функциях. Иммунная привилегированность ЦНС не абсолютна. В норме экстравазация лимфоцитов через ГЭБ имеет решающее значение для иммунного надзора в нервной ткани, хотя и жестко регулируется. Повреждение ГЭБ является одним из центральных звеньев в патогенезе многих заболеваний нервной системы, а неспецифическая проницаемость ГЭБ может существенно возрастать при различной соматической патологии. В целом иммунологическая дисфункция и воспаление — основные факторы в нарушении целостности ГЭБ. Механизмы метастазирования злокачественных лимфом в ЦНС остаются малоизученными, хотя уже сейчас понятно, что для их осуществления необходимы взаимодействия между циркулирующими опухолевыми клетками и компонентами ГЭБ, при этом некоторые цитокины могут действовать как аттрактанты для метастазирующих клеток, а процесс развития опухоли включает в себя несколько этапов от экстравазации до локальной пролиферации и активации неоангиогенеза.</p></abstract><trans-abstract xml:lang="en"><p>The review summarizes modern researches about the role of the blood-brain barrier (BBB) in the development of lymphomas of the central nervous system (CNS). Currently, it has been established that the BBB is a highly active structure. On the one hand, it protects the brain tissue from chemical, physical and other infl uences, and on the other hand, it is uniquely adapted for transmitting signals between the CNS and other parts of the body. BBB cells respond to signals from the CNS or blood compartments that can stimulate changes in their barrier, transport, and secretory functions. The immune privilege of the CNS is not absolute. Normally, extravasation of lymphocytes through the BBB is crucial for immune surveillance in nervous tissue, although it is tightly regulated.</p><p>Damage to the BBB is one of the central links in the pathogenesis of many diseases of the nervous system, and the nonspecifi c permeability of the BBB can increase signifi cantly in various somatic pathologies. In general, immunological dysfunction and infl ammation are the main factors in the violation of the BBB integrity.
