Поиск зависимостей между клиническими особенностями пациенток с миомой матки и характеристиками их миоматозных узлов актуален для выявления прогностических маркеров.</p></sec><sec><title>Цель</title><p>Цель . У пациенток репродуктивного возраста с миомой матки выявить наличие корреляций между клинико- лабораторными показателями и маркерами ангиогенеза и фиброза в операционном материале.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы . Проведена миомэктомия у 226 пациенток с наличием миоматозных узлов в теле матки. Обследование пациенток включало в себя инструментальные методы (ультразвуковое исследование, магнитно-резонансная томография), изучена экстрагенитальная патология. Образцы доминантных миоматозных узлов и миометрия были исследованы с помощью иммуноферментного анализа (ИФА) на содержание в среде инкубации базового и стимулированного поликлональными активаторами (ПА) фактора роста эндотелия сосудов (VEGF) и гранулоцитарно-макрофагального колониестимулирующего фактора (ГМ-КСФ). Изучали экспрессию прогестероновых рецепторов (PgR) и VEGF с помощью иммуногистохимии (ИГХ).</p></sec><sec><title>Результаты</title><p>Результаты . Сочетание миомы матки с эндометриозом у женщин репродуктивного возраста влияло на ряд гистологических и молекулярных характеристик доминантного миоматозного узла: положительно – на степень фиброза (р &lt; 0.0001), отрицательно – на индуцированную ПА продукцию ГМ-КСФ и VEGF (р = 0.0054 и р = 0.0016 соответственно). У пациенток с аденомиозом было больше миоматозных узлов (р = 0.0192) по сравнению с пациентками без любой формы эндометриоза, при этом наибольший диаметр узла и степень его фиброза были у женщин без аденомиоза (р = 0.0397 и р = 0.0344 соответственно). Индуцированная ПА продукция VEGF в узле больше при наличии адено- миоза (р = 0.0326), а экспрессия PgR в перифокальном миометрии – меньше (р = 0.045). У пациенток с миомой матки индекс массы тела слабо положительно коррелировал с экспрессией VEGF в перифо- кальном миометрии. Количество миоматозных узлов слабо положительно коррелировало с индексом влияния поликлональных активаторов (ИВПА) на продукцию VEGF в доминантном узле in vitro. Степень фиброза доминантного узла умеренно положительно коррелировала с экспрессией VEGF в перифокальном миометрии по данным ИГХ.</p></sec><sec><title>Заключение</title><p>Заключение . Результаты исследования отражают системность миоматозной трансформации с мульти- фокальностью опухолевого роста миометрия и потенциалом образования новых миоматозных узлов у пациенток с повышенным индексом массы тела.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>I n t r o d u c t i o n</title><p>I n t r o d u c t i o n . The search for the relationships between clinical features of patients with uterine fi broids (uterine myoma, leiomyoma) and the characteristics of their myomatous nodules is relevant for identifying prognostic markers.</p></sec><sec><title>A i m</title><p>A i m . In patients of reproductive age with uterine fi broids, to identify the presence of correlations between clinical laboratory parameters and markers of infl ammation and fi brosis in the surgical specimens.</p><p>M a t e r i a l s  a n d  m e t h o d s . Myomectomy was performed in 226 female patients with myomatous no dules. Examination of patients included clinical investigations (ultrasound, magnetic resonance imaging), extragenital abnormalities were assessed. Samples of the dominant myomatous nodules and myometrium were examined using enzyme-linked immunosorbent assay (ELISA) for the content of baseline and stimulated by polyclonal activators (PA) vascular endothelial growth factor (VEGF) and granulocyte-macrophage colony-stimulating factor (GM-CSF) in the incubation medium. The expression of progesterone receptors (PgR) and VEGF using immunohistochemistry (IHC) was assessed.</p></sec><sec><title>R e s u l t s</title><p>R e s u l t s . The combination of uterine fi broids with endometriosis in reproductive-aged women infl uenced a number of histological and molecular characteristics of the dominant myomatous nodule: positively – on the grade of fi brosis (p &lt; 0.0001), negatively – on the PA-induced production of GM-CSF and VEGF (p = 0.0054 and p = 0.0016, respectively). Patients with adenomyosis had more myomatous nodules (p = 0.0192) compared with patients without any form of endometriosis, while the largest nodule diameter and the degree of fi brosis were in women without adenomyosis (p = 0.0397 and p = 0.0344, respectively). PA-induced VEGF production in the nodule is greater in adenomyosis (p = 0.0326), while PgR expression in the perifocal myometrium is less (p = 0.045). In patients with uterine myoma, the body mass index had a weak positive correlation with VEGF expression in the perifocal myometrium. The number of myomatous nodules had weak positive correlation with the stimulation index of PA (SIPA) which refl ects their infl uence of the VEGF production in the dominant nodule in vitro. The degree of fi brosis of the dominant nodule was characterized by moderately positive correlation with the expression of VEGF in the perifocal myometrium according to IHC.</p></sec><sec><title>C o n c l u s i o n</title><p>C o n c l u s i o n .
