Молекулярно-клеточные механизмы формирования недифференцированных форм дисплазии соединительной ткани (обзор литературы)

Аннотация

Введение . Дисплазии соединительной ткани включают недифференцированные формы (НДСТ) и гипермобильный тип синдрома Элерса – Данлоса (ГТСЭД), их частота составляет около 1:500. Современные лечебнодиагностические протоколы основаны на клинических аспектах и не отражают патогенетические особенности этих состояний. Новое понимание механизмов дезорганизации соединительной ткани открывает перспективы поиска лекарственных мишеней и лабораторных маркеров.

Цель . Актуализировать схему патогенеза НДСТ и ГТСЭД для обоснования выбора потенциальных мишеней и методов диагностики.

Материалы и методы . Проведен поиск рецензируемых публикаций за период 2012–2025 годов в базах PubMed, Scopus, Google Scholar и eLIBRARY.

Результаты . Транскриптомные исследования не выявили значимых различий между НДСТ и ГТСЭД: снижена экспрессия 826 генов (репарация ДНК, цитоскелет), повышена – у 126 генов (протеазы, провоспалительные цитокины). Исключение – полиморфизмы MTHFR, разная частота аллелей при НДСТ и ГТСЭД не подтверждена на выборках. В клеточных культурах выявлены общие ключевые механизмы, в частности преобладание миофибробластной дифференцировки через TGF-β- и интегрин-зависимые пути. Нарушенная синтетическая активность миофибробластов приводит к хаотичному расположениюколлагена I, III и V типов, уменьшениюпоперечных сшивок и внутриклеточному накоплению эластина. Это вызывает разрушение внеклеточного матрикса (ВКМ) с образованием фрагментов – фибронектина массой 52 кДа и коллагена I типа массой 45 кДа. Нарушение структуры ВКМ приводит к уплотнению фасций и вывихам. На основе выявленных механизмов определены потенциальные лекарственные мишени: ингибиторы матриксных металлопротеиназ (ММР) (доксициклин), коррекция фолатного цикла (5-метилтетрагидрофолат), блокада TGF-β сигналинга (целипролол).

Заключение . Значимых различий геномного профиля и метаболической активности клеток между НДСТ и ГТСЭД не выявлено. Эти состояния представляют собой единый процесс дезорганизации соединительной ткани. Главный патогенетический механизм – порочный круг активации миофибробластов, дезорганизация ВКМ, активация иммунных клеток и хроническое воспаление, которое дополнительно стимулирует миофибробласты. Патогенетически обоснованными терапевтическими подходами являются ингибирование ММР, коррекция фолатного цикла и блокада TGF-β. Соответствующие молекулы (доксициклин, 5-метилтетрагидрофолат, целипролол) требуют клинической валидации.
