Abstract
The dynamics of the pain syndrome was studied in 108 patients with multiple myeloma complicated by peripheral neuropathy due to bortezomib injections. All patients were divided into 3 groups: in the 1st group standard polychemotherapy was supplemented with a course of dynamic electrical neurostimulation; in the 2nd one standard treatment was carried out against the background of imitation of a similar procedure from a placebo apparatus; in the 3rd group only standard treatment was administered. By the end of the course of therapy, the severity of the pain syndrome decreased in patients in the 1st group by 1.8 times, in the 2nd and 3rd groups by 1.3 and 1.2 times respectively. The initially increased threshold of pain sensitivity decreased in the 1st group by 24.4 %, whereas in the 2nd and 3rd groupes by 8.3 and 8.1 % respectively. Consequently, the inclusion of dynamic electrical neurostimulation in the treatment of patients with multiple myeloma accompanied by drug-induced peripheral neuropathies reduces the severity of pains and decreases the threshold of pain sensitivity allowing the continuation of specific polychemotherapy.

Abstract
146 HIV-infected patients with acute meningoencephalitis of various etiologies took part in a complex clinical and immunology research. It was determined the efficiency of combined use of citicoline antioxidant and polyoxidonium immunomodulator in complex treatment of this contingent of patients. It was proved the positive effect of the use of this regimen of the treatment of cerebral, meningeal syndrome complexes, focal neurological disorders in a shorter period in comparison with traditional therapy, as well as in improvment the indices of cellular immunity.

Abstract
We analyzed the communication features of 12 patients with borderline personality disorder during long-term psychodynamic psychotherapy. A high sensitivity in the relationships between patients and a psychotherapist and also sudden patients’ mental changes during analytical sessions were revealed. These features are considered to be the possible traps for a psychotherapist in a way making a therapy rather difficult. Regression states arising in patients during analytical sessions which reach the level of «basic deficiency» are described. In these cases the therapy’s efficiency proved to be connected with psychotherapist’s ability for psychological regress to the patients’ mental level. We substantiated the necessity of the psychotherapist for being ready to form a countertransference which includes a response to emergence of different alters in the dissociate patients’ states.

Abstract
The character and structure of inflammatory pathology in 5225 patients aged from 3 up to 14 years who were treated in Maxillofacial Surgery Department in Municipal Children’s Hospital of Emergency Medicine (Novosibirsk) in 2009-2014 are studied. The frequency of diseases of the jaws, sialaden, phlegmons and abscesses of an oral cavity, furuncles, lymphadenites during the specified period changed slightly. But the number of patients with exacerbation of chronic periodontitis increased sharply: from 158 (9.2 %) cases in 2009 to 621 (36.9 %) cases in 2014. Evident increase or depression is not registered among inflammatory diseases of jaw-bones, and the acute serous and purulent periostitis made 92 % of all pathology. Also he augmentation in number of diseases sialaden was not observed. Serous sialadenitis (68 %) prevailed among this pathology, purulent sialadenitis was fixed in 28 % of cases. The lymphadenitis is diagnosed in 17.7 % of patients, at the same time acute serous lymphadenitis was revealed in 539 patients, and purulent one - in 333 patients. Treatment of serous lymphadenites in children using effective methods of antibacterial and immunomodulatory therapy resulted in recovery without surgery. It is our opinion that increase of chronic periodontitis revealed during the conducted research is caused by impossibility of providing full surgical care in out-patient conditions due to the lack of the general anesthesia, and the high level of carious lesion of both temporary, and permanent teeth in children that demands promotion of healthy lifestyle in organized children’s formations, among medical personnel, tutors, teachers, parents and improvement of methods of stomatological prophylaxis.

Abstract
Benign paroxysmal positional vertigo (BPPV) is the most common form of vestibular dysfunction associated with marked distress and a decrease in the quality of life of patients. The purpose of the study was to improve the MRI imaging of the semicircular canals in the norm and in benign paroxysmal positional vertigo. Magnetic resonance imaging was performed in 80 patients (images of semicircular canals of 160 temporal bones were obtained). Among them, there were 65 patients with vertigo and 15 patients without vestibular symptoms. All patients underwent standard brain MRI on a magnetic resonance tomograph SIGNA EXCITE HD 1.5T Twin Speed (GE, MEDICAL SYSTEMS), supplemented with a pulse sequence of 3D FIESTA-C. The most common symptom in patients with BPPV was point defects of filling in the right posterior semicircular canal. It is advisable to include MRI in the diagnostic algorithm for patients with vertigo along with positional techniques and traditional methods of examining the vestibular analyzer. Examination of patients with vestibular vertigo using the pulse sequence (3D FIESTA-C) is more accurate than the standard MRI technique and provides important diagnostic information.

Abstract
Exudative otitis is one of the most common diseases of the middle ear. To date, the clinical picture of this disease is often dim, which makes it difficult to diagnose and provide timely treatment. Traditional therapy includes systemic and local decongestants, while this paper demonstrates the clinical efficacy of intranasal glucocorticosteroids in the treatment of this pathology, since the etiopathogenesis of this type of otitis is based on the development of nasal obstruction caused not only by the vasomotor, but also by the allergic component. The results obtained make it possible to recommend a scheme for the complex treatment of exudative otitis using local treatment, as well as systemic anti-inflammatory, desensitizing, mucolytic and decongestant therapy for wide practical use in otorhinolaryngological practice.

Abstract
Influence of the instrumental rehabilitative techniques in 55 patients operated concerning congenital or acquired defects of the nose divided into 2 groups is studied: physiotherapeutic technologies were applied in preoperative, early and late postoperative periods in the 1st group, the 2nd one was performed only surgery. In patients of the 1st group instrumental physiotherapy promoted decrease of the area of ecchymoses to the end of the early postoperative period by 3.4 times, the full stopping of a pain syndrome, augmentation of thickness of dermal layer in 13.4 %, filling of hypoechoic zones in derma with structural elements with augmentation of density of dermal layer by 1.4 times in 30 days after the operation that wasn’t observed in the 2nd group.

Abstract
Data on dynamics and structure of extrapulmonary tuberculosis (TB) case rate in 2000-2016 in TB epidemiologically negative region, the Republic of Tajikistan are submitted. The extrapulmonary TB case rate for the analyzed period fluctuated from 6.7 to 11.2 cases per 100 thousand population. The number of the patients with extrapulmonary TB from 2000 to 2010 increased steadily in Tajikistan (from 412 people in 2000 up to 837 people in 2010 that is twice that size). The number of such patients reduced to 665 by 2016. Tuberculous pleuritis, osteal and joint TB and lymphatic TB prevailed in the case rate structure. The share of genitourinary TB has increased considerably since 2010, and the share of TB in other localizations, mainly abdominal has increased considerably since 2014 too. Thus, the problem of extrapulmonary tuberculosis in Tajikistan looms large, and its solution is connected with providing the specialized diagnostic equipment in laboratories of the regions of the Republic and training of highly skilled medical personnel.

Abstract
In order to find the relationship between the accentuations of personality traits identified by K. Leonhard and personality disorders, 90 students of the 4th and 5th courses of two different universities of Novosibirsk (Novosibirsk State Medical University - 45 people, 1st group and Siberian Transport University - 45 people, 2nd group) were examined for 20 men and 25 women in each group at the age from 23 to 26 years. It was found that certain relationships between the accentuations of personality traits and personality disorders exist, and narcissistic tendencies are manifested in all the accentuations revealed in the respondents. In addition, it was determined that medical students are render liable to personal disorders than railway students.

Abstract
Network theory of mental disorders is a fairly new research. Its appearance is connected with methodological problems that arose when determining the pathogenesis of mental disease. The network involving statistical detection of network trends from empirical data is being applied and developed within the sociodynamic paradigm of psychiatry thus providing new opportunities to treat mental pathology in the post-modern period.
